Insurance Information
As a courtesy to our patients, we will be happy to assist you in filing claims with your insurance carrier. We are not responsible for the amount your carrier pays which may be less than your actual charges. You are responsible for all charges incurred including those covered by insurance.
Here at Fountain Head Dental we accept the following carriers (In Network):
- Blue Cross Blue Shield, Delta Dental, and Cigna PPO
Payment Information
Payment for services are due at the time services are rendered. We accept cash, checks and most major credit cards. Ask our financial manager for details about our financial arrangements, which include 6 to 12 months same-as-cash for charges of $300 or more through Care Credit.
